Services Provided
EdiGrace Social Services provides geriatric healthcare consulting, care coordination, and social work counseling services. Services are designed to support elderly clients and their families in navigating healthcare systems, improving quality of life, and ensuring appropriate care planning
Client Responsibilities
Clients and/or their representatives are responsible for providing accurate and complete information, participating in care planning, and communicating any changes in condition or needs promptly.
Payment Policy
All services are private pay unless otherwise stated. Payment is due prior to or at the time of service unless a written agreement is in place. Accepted payment methods will be outlined during onboarding.
Cancellation & Rescheduling
Clients must provide at least 24 hours notice for cancellations or rescheduling. Late cancellations may be subject to a fee.
Confidentiality
All client information is kept confidential in accordance with applicable privacy laws and professional standards. Information will only be shared with authorized parties or as required by law.
Scope of Practice
EdiGrace Social Services does not provide emergency medical services. In case of emergency, clients should call 911 or seek immediate medical attention.
Communication Policy
Communication may occur via phone, email, or in-person visits. Response times may vary, especially outside of standard business hours.
Liability Disclaimer
EdiGrace Social Services is not liable for outcomes resulting from third-party providers or client decisions. Services are advisory and supportive in nature.
Termination of Services
Services may be terminated by either party at any time with written notice. Any outstanding balances must be paid in full upon termination.
Agreement
By engaging in services, the client acknowledges and agrees to these terms and conditions.
